Appointments, Cancellations, and Payment

• Cancellation, rescheduling, deposit, and no-show policies may apply and will be communicated to you at the time of booking or during your consultation.

The booking window is operated by Boulevard, and anything you enter into it is handled under Boulevard’s own terms and privacy practices. An embedded Google Map displays our location and provides directions. The map is provided by Google and is subject to Google’s own terms.

Patient financing, where offered, is provided by third-party lenders. Application, approval, interest, and repayment terms are determined by and agreed between you and that lender. Identity Aesthetics MedSpa, LLC is not a lender and is not a party to that agreement.
